In order to add your child to The Halo Garden's Wall Of Fallen Heroes, simply fill out the following Memorial Wall Submission Form below.

If the following submission form does not appear to be in working order, please send us an email at halogarden@columbus.rr.com. In your email, please enter "My Hero" in the subject line and provide the information that is requested on our form.

All children who are added to The Halo Garden's Wall Of Fallen Heroes are automatically added to The Halo Garden Of Dates To Remember. If for some reason, an oversight has been made, and your child is not listed on this page, please send us an email so that we may correct this ASAP!

We would also like to dedicate a butterfly in memory of your child. Prior to completing the below form, would you please first choose a butterfly that you would like dedicated to your child? We have several for you to choose from and you can make a selection by clicking here. If you choose a butterfly on our Memorial Wall Submission Form, there is no need to complete the submission form found on our Select A Butterfly page.

Unless otherwise stated, all comment fields on our submission form are required. In order to ensure that we are able to include your child without delay, please be sure to completely answer each field.

All submissions to The Halo Garden will be added within 2 weeks from the date that your request was received.
